2022 METRIC GUIDELINES:

We have enrolled University of New Mexico Family Medicine in the METRIC Hypertension: Improving Patient Care Module.

Your Provider Group ID Number is: NMAL1583

Start Date: 02/14/2022

End Date: 12/31/2022

Please be aware the current METRIC modules will expire on December 31, 2022. The module must be completed by this date to receive credit. 

Please share the following information with your group members before their initial log-in to the METRIC module(s):

1. Log into your AAFP account.
2. Click “Welcome” in the top corner.
3. Under “Online Access” select your group module.
4. Complete the registration page. 
5. Continue to the next page & select yes to participate in the module as a group member. 
6. Enter Provider Group ID number: NMAL1583

(This Provider Group ID Number needs to be entered exactly as shown for the participant to be included in the group.)

Guidelines:

• The reassessment/intervention implementation timeout period is one month. Please plan accordingly.
• Read all directions carefully.
• In order to move forward in the module, the Continue button must be clicked.
• Chart Review requires 10 patient charts to be entered.
• Once your data is submitted, you will not be able to return to prior steps and make changes.
