Backup: Inpatient and Continuity support

Background

In order to support all residents when they are sick or are not available for emergent reasons, backup support is necessary.  All of you will likely need backup support at least once during your residency and all of you will have to be on backup at least once.  We hope that no one will over-utilize, or unfairly utilize, backup coverage.  In order to ensure equitable use of backup for everyone,  if while on backup you have to cover more than two 12-hour shifts for the same resident during your 2 week rotation, the third shift and beyond that is covered for the same resident will have to be "paid back" in kind.  The resident who required backup and the backup resident will work out between them a call change that works for both of them, please then review with Dolores to ensure that no work hour rules are violated by the exchange. 

 

THE EXPECTATION FOR THIS ROTATION IS THE SAME AS ANY OTHER INPATIENT ROTATION IN TERMS OF HOURS YOU MAY HAVE TO WORK.  OFTENTIMES THE BACK UP RESIDENT IS PROVIDING BACKUP FULL TIME DURING THE TWO WEEKS, INCLUDING 24 HOUR SHIFTS, NIGHTS ETC.  THE BACKUP RESIDENT IS NOT TO VIOLATE WORK HOUR RULES HOWEVER:

-NO MORE THAN 80 HOURS A WEEK

-10 HOUR BREAK AWAY FROM WORK AFTER 12 HOUR SHIFT

-14 HOUR BREAK AWAY FROM WORK AFTER 24 HOUR SHIFT

-ONE DAY OFF IN SEVEN [AVERAGED OVER 4 WEEKS]

 

Rotation Instructions

Please read the FM Resident Backup Inpatient and Continuity Support Procedures document that is attached below for the specific expectations for this block.

 

Continuity Clinic Procedure  (highlighted here because this procedure is different than usual clinic) 

The Backup resident will still need to be in clinic to meet clinic requirements. Usually, 2 clinics per week have been scheduled on Thursday and Friday afternoons. Residents should make all reasonable attempts to make it to clinic using the clinic-specific guidelines, though should not break duty hours. This means if a resident is called in at night and will have fewer than 8 hours before the start of afternoon clinic, they should cancel that afternoon clinic. Otherwise, you should be prepared and dressed to see patients at the scheduled times.

UNM North Valley

Contact Kimmie McKinney- Unit Director by Tiger or Cell 505-321-6885 (usually first thing in the morning if you're not called in).  At NVC, your Thursday clinic is in FOCUS. Friday clinic is for acute visits. You are expected to contact Kimmie to open an acute care clinic if you know you will be able to be in clinic that day.

 

UNM Southeast Heights

Contact charge nurse Samantha Watson (Tiger) and Ryan Harris 505-697-9638 as soon as you know you will be available for clinic (usually first thing in the morning if you're not called in). You are expected to contact them to open an acute care clinic if you know you will be able to be in clinic that day. Please message them with timely updates that you will be in clinic (with updates if changes to availability).

 

First Choice South Valley

Residents should text Dr. Cheruvu (505-818-7087) and send a Teams message

 

 

Practice Management modules

The lighter clinical schedule during this block allows residents to meet some of the other GME training requirements in Practice Management in the form of some self-guided modules (on HSC Moodle site) and faculty mentorship.  Residents will be scheduled for a meeting with supporting faculty at the beginning and the end of this block to review targeted modules and personal goals.  

 

The upcoming meeting dates (generally 1st Wed and 2nd Friday, 11am-12pm, but may vary depending on faculty availability) are on your Outlook calendar:
